Work hours will be days, Monday - Friday, for 80 hours per pay period, and as needed.
This role provides care management and population health services to patients in the Dubuque Region. The Cross Continuum Care Manager will be responsible to create longitudinal, personalized care plans for patients/family/support system and to collaborate with and coordinate the efforts of the interdisciplinary care team across the continuum. In addition, this role will require the ability to utilize data analytics to manage the health of populations to improve access to care and clinical outcomes.
This role requires someone who is innovative, has strong self-motivation, ability to work independently, and has the ability to collaborate well with others in various care settings.
Minimum of three years of clinical nursing experience required with experience in a clinic, home care setting, and/or care coordination and population health experience preferred. BSN preferred.
