We are looking for a full-time, Social Worker (BSW), to work in our Social Worker department. Work hours will be days, Monday - Friday, for 64 hours per pay period.
The social worker role screens patients to identify psychological, social, financial, and behavioral issues and works collaboratively with the interdisciplinary care team in the development of the plan of care for the patient. Serves as a patient advocate, provides ongoing psychosocial support and facilitates access to supportive, rehabilitative, palliative care services, and other resources for patients and families throughout the care continuum. Assists patient in establishing patient-centered goals of care incorporating psychosocial and behavioral strengths and weaknesses. Enables informed decisions.
A Bachelors Degree in Social Work is required. Licensure from the Iowa Department of Public Health (via the IowaBureau of Professional Licensure) is preferred.
