About the Program

The Certified Community Behavioral Health Clinic (CCBHC) aims to provide a comprehensive range of mental health and substance-use disorder services, with an emphasis on helping vulnerable individuals.  These services include:

  • Crisis mental health services
  • Outpatient mental health and substance-use services
  • Screening, assessment, and diagnosis
  • Patient-centered treatment planning


This program will benefit uninsured, underinsured, and underserved residents in Dubuque and Jackson counties, by offering multiple services in a single location and creating a more accessible and seamless environment for clients.  The clinic’s goal is to remove barriers and improve access to care and reduce wait times to give our clients quicker access to services.



Your Responsibilities

As a LISW you will provide therapy to individuals, children, groups and families to promote optimum brain health. An LISW will utilize their unique skills and treatment modalities to meet the needs of the individual requesting services. The LISW will work with individuals with Obsessive Compulsive Disorders, Depression, PTSD, Adjustment Disorders, ADHD, Bereavement, Anxiety Issues, Phobias and other persistent brain health issues. Responsibilities will also include:

  • Completing an initial assessment, diagnostic summary, and other assessments as required
  • Developing a person-centered treatment plan
  • Completing progress notes within 24 hours of service delivery
  • Discharge planning and make referrals for additional services as needed
  • Collaborating with other professionals involved with clients, when appropriate



What You Need

  • Master’s degree in social work
  • Current and active licensure as a Licensed Independent Social Worker
  • Effective counseling and diagnosing skills, and a proficient knowledge of the impact of trauma
  • Proficiency in time management and EHR documentation
  • Experience working with the designated population and sub-populations
  • Demonstration of a professional commitment to providing services to medically under-served individuals
  • Must be able to perform the essential functions of this position
