Dubuque Finley Home Care
Schedule: Work hours will be days for 80 hours per pay period. There are rotating weekends and holidays. There is some call.
Duties and Responsibilities:
Provide professional nursing care in the home by utilizing all elements of the nursing process. Collect health data, demonstrate knowledge and application of patient assessment skills, perform initial and ongoing assessment of patient and family/caregiver. Analyze the assessment data to determine nursing diagnosis and develop plan of care. Implement the interventions identified in the plan of care in a safe, timely, and appropriate manner. Evaluate the patient’s progress toward attainment of outcomes on an ongoing basis. Complete all required documentation accurately, thoroughly, and on time. Educate and involve patients, families, and caregivers to promote self-care activities. Coordinate care with other disciplines and provide supervision of nursing assistants. Identify and coordinate community resources and reimbursement resources necessary to support the plan of care. Promote continuity of care through discharge planning, case management, and advocacy.
Requires graduate of approved school of nursing, active Iowa RN licensure issued by the Iowa Board of Nursing, history of consistently demonstrated excellent customer service, quality of care, interpersonal and team work skills that meet or exceed the Finley Hospital core values, service excellence and promise to my co-workers expectations. Within 1 year of hire, need to obtain RN Licensure with the State of Wisconsin and Illinois. CPR certification is required. BSN degree preferred. Must have valid driver's license and proof of personal auto insurance coverage.
