UnityPoint Health Finley Hospital is a 126-bed community hospital. We employ more than 800 full-and-part-time employees. Finley Hospital is a not-for-profit, locally owned, regional healthcare facility part of the UnityPoint Health system. Work hours will be days, for 80 hours per pay period, and as needed. You will be required to flex to other shifts. There is a hoiday and weekend rotation.
This role develops and coordinates the overall plan of care for assigned patients throughout their hospital stay. The plan of care may include managing the transitions of care to and from the hospital setting. Establish strong partnerships with patients, families, physicians and all members of the care team. Assume a broad level of accountability for the outcomes of care for their assigned patients and/or populations. Provide leadership to the multi-disciplinary team. Manage performance improvement projects as assigned.
Active Iowa RN License issued by the Iowa Board of Nursing. Must have a Bachelor's Degree in Nursing or must obtain BSN within 5 years of hire date.
