Consistent with the American Nurses Association Standards of Clinical Nursing Practice and Scope of Practice for the Palliative Care Nurse Practitioner (PCNP), the PCNP provides comprehensive, holistic advanced nursing care to patients' chronic, progressive disease. The PCNP functions in four areas of advanced practice: 1) direct, comprehensive patient care, 2) systems management, 3) education, and 4) professional leadership and publication. The PCNP conducts all phases of case management for patients with chronic, progressive disease including assessment, close collaboration with physicians, use of protocols to manage routine patient problems and coordinating the recommendations of various disciplines. The PCNP will use advanced knowledge and skill to identify abnormal conditions, diagnose health problems, develop and implement treatment plans, evaluate patient outcomes, and collaborate with the physicians and the multidisciplinary team to deliver patient-centered palliative care services to patients. In addition to the clinical practice, the PCNP serves as consultant, mentor/role model, organizational leader, and is involved in program development and implementation, including, but not limited to, active participation / facilitation of palliative care and chronic disease management initiatives.
Customers include: patients, families/visitors, physicians, physician assistants, nurse practitioners, nurses, case managers, clinical documentation specialists, coders, clinical professional services associates, representatives of third party payers, representative of referring agencies, interdepartmental and intradepartmental associates, vendors, and volunteers.
Master of Science in Nursing (MSN) required.
Current ARNP licensure in the State of Iowa, Illinois and Wisconsin
Child and Dependent Adult Abuse Mandatory Reporter Training is required within 6 months of hire and every 5 years thereafter.
Basic Life Support Certification (BLS) will be required within 30 days of hire and every 2 years thereafter.
3-5 years clinical nursing experience with a minimum of 2 years of nurse practitioner experience in the hospital or clinic setting.
Experience with chronic disease management, hospice, or palliative care preferred.
Demonstrated leadership and management abilities preferred.
Experience with electronic medical records (such as Cerner/PowerChart) and computer applications such as Microsoft Office Suite (Word, Excel, PowerPoint, Outlook), HealthStream, Intranet; and use of reporting tools.
