ABOUT OUR HOSPITAL
UnityPoint Health Finley Hospital is a 126-bed community hospital. We employ more than 800 full-and-part-time employees. Finley Hospital is a not-for-profit, locally owned, regional healthcare facility part of the UnityPoint Health system. Finley Hospital is committed to bringing the best medical care to the tri-state area. We offer comprehensive, full service health care that is coordinated around our patients. Work hours will be days, Monday through Friday, for 32 hours per pay period, and as needed. There is a weekend rotation. There may be a holiday rotation in the future.
This role develops and coordinates the overall plan of care for a cohort of patients throughout their hospital stay. The plan of care may include managing the transitions of care to and from the hospital setting. Establish strong partnerships with patients, families, physicians and all members of the care team. Assume a broad level of accountability for the outcomes of care for their assigned patients and/or populations. Provide leadership to the multi-disciplinary team. Manage performance improvement projects as assigned.
Qualified candidates must have an active, Iowa RN licensure issued by the Iowa Board of Nursing. Candidates must have a BSN or be currently enrolled in a BSN program with the expectation to complete the program within 5 years of hire. Candidates must have a minimum of 3 to 5 years of RN experience at the acute care, intermediate care or home care setting. A strong clinical background is preferred. Knowledge of utilization review, case management and performance improvement experience is preferred.
