Serves as the frontline resource on issues that arise in the day-to-day spectrum of Home Care. Acts as a liaison to the medical staff, other hospital department associates, and outside referral sources.
Provides leadership in the areas of problem solving, and troubleshoots issues in the patient care areas. Keeps the director apprised of issues as appropriate.
An MSW from an accredited school of Social Work required. Social Work experience in hospital preferred. Social Work licensure strongly preferred. Child and Dependent Adult Abuse Mandatory Reporter Training is required within 6 months of hire and every 5 years thereafter.
Will need to obtain an Illinois and Wisconsin license.
