About the Program
The Assertive Community Treatment (ACT) program is an effective, evidence based treatment program to provide in-home treatment, support, and rehabilitative care to persons with serious mental illnesses.
ACT's wraparound services aim to reduce the number of people who are in and out of hospitals for treatment or who are incarcerated or in residential care facilities. This is done mobilizing mental health and treating people out in the community. ACT teams are made up of experts, such as psychiatrists, psychologists, social workers, and employment specialists, who are on call 24 hours a day, seven days a week.
As a Community Support Specialist, your primary responsibility includes providing case coordination, skills teaching, counseling, crisis intervention and support services to consumers assigned to the ACT team. In addition, you will:
- Coordinate and provide habilitation services
- Provide individual supported employment services to, or on the behalf of, the member that enable the member to obtain and achieve stabilization in individual employment
- Conduct assessments and creates service plans based on client needs, wants and abilities in a timely manner, as required
- Communicate and coordinate services with support persons/guardians and other providers on a regular basis as appropriate
What you need
- Bachelor's degree with at least 30 semester hours or equivalent quarter hours in a human services field, including but not limited to sociology, social work, counseling, human services or psychology
- Experience working with persons with severe and persistent mental illness
- Must be eligible to drive agency vehicles
