MercyOne Home Care (certified/regulated) serves patients referred at discharge from a hospital or nursing home, or from a physician office. Services include nursing, certified assistant nursing (aide), physical and occupational therapy, and medical social work. The main office is located in Dubuque, Iowa, and a branch office is located in Galena, Illinois. Nursing services include medical/surgical, as well as wound/enterostomal, maternal child, and pediatrics. Clients range in age from newborn to the frail elderly, with the majority being of Medicare age. Service is provided to the tri-state area within an approximate 35-mile radius from Dubuque. Requirements for services: • The client must meet the criteria of Medicare, Medicaid, and insurance companies. • The client must be under the care of a physician. • The client must have a skilled, intermittent need. Nurses work 8-hour shifts with some call and every 4th weekend rotation. One year of nursing experience is required. We hire both ADN and BSN (preferred). Orientation is 12 weeks with a preceptor and on-going support with case management from office staff. CNAs provide bathing services to clients in the home. Office staff are comprised of medical coder, transitional care coordinators, quality improvement coordinators, scheduler, and medical/secretarial support. MercyOne Home Care office hours are Monday – Friday, 0800 – 1630.
