Patient Care Coordinator - Medical - Full Time Days

Consistent with the American Nurses Association Scope and Standards of Practice (2004), the Patient Care Coordinator is a professional nurse and a member of the Mercy Medical Center healthcare team and is accountable for the delivery of holistic, compassionate, individualized, and effective patient care demonstrated by the use of the nursing process: assessment, diagnosis, outcome identification, planning, implementation, and evaluation. In addition, the Patient Care Coordinator is responsible for behaviors consistent with the professional nursing role, appropriate to their education and experience, including activities related to quality of care, performance appraisal, education, collegiality, ethics, collaboration, research and evidence-based practice, and resource utilization. The Patient Care Coordinator facilitates interdisciplinary team planning to maximize patient outcomes throughout the continuum of care and serves as a clinical resource for the defined patient population. The Patient Care Coordinator assumes accountability for the nursing department in the absence of the Director.

•   must hold an active Registered Nurse license to practice in the state of Iowa
•    a Bachelor of Science in Nursing (BSN) is required within 4 years of hire (tuition assistance provided as an employment benefit)
