About the Program

At Hillcrest, we strongly advocate for the wellness of our clients through our Integrated Health Home (IHH) Program that delivers the following services, and more, using an individual/family-centered, strengths-based approach. For children and adults, our services include:

  • Treatment of particular mental illnesses including anxiety, depression, bipolar syndrome and ADHD
  • Comprehensive care management and coordination of our client's health including coordination of doctors, therapists and prescribers
  • Health promotion and wellness prevention activities
  • Comprehensive transitional care
  • Individual and family support services
  • Referral to community and social support services
  • A Hillcrest team of nurses, care coordinators and peer group specialists to make certain all wellness needs are being taken care of Integrated Health Homes "expand the traditional medical home models to build linkages to other community and social supports, and to enhance coordination of medical and behavioral healthcare, in keeping with the needs of persons with multiple chronic illnesses." Your Responsibilities

As a Recovery Support Advocate , your primary responsibility includes encouraging clients to take control of their own health and wellness and to promote the concept of "Whole health"  . In addition, you will:

  • Understands and demonstrates the ability to welcome persons and families to IHH and related programs. Understands the philosophy and purpose of the IHH program and demonstrates the ability to effectively educate others on the value of the program
  • Demonstrates the knowledge and purpose of funder’s processes and requirements as it relates to the enrollment of new IHH Members.
  • Provides recovery support through lived experience, awareness, and commitment to the understanding of Mental Health, to persons in recovery and their family members
  • Becomes familiar with community resources to assist care coordinators in connecting IHH clients to needed services.

What You Need

  • High school diploma or equivalent with two or more years of experience
  • Lived experience, in recovery and models recovery.   Be able to provide a written story about the lived experience and recovery.
  • Graduate Iowa Peer Support Training Academy or equivalent within 1 year of hire.
  • Valid driver's license and the ability to drive agency vehicles.
  • Ability to work with individuals with a lived experience of mental health or substance use conditions.
  • Adheres to the Peer Support Specialist Code of Ethics.
