About the Program
The Certified Community Behavioral Health Clinic (CCBHC) aims to provide a comprehensive range of mental health and substance-use disorder services, with an emphasis on helping vulnerable individuals. These services include:
- Crisis mental health services
- Outpatient mental health and substance-use services
- Screening, assessment, and diagnosis
- Patient-centered treatment planning
This program will benefit uninsured, underinsured, and underserved residents in Dubuque and Jackson counties, by offering multiple services in a single location and creating a more accessible and seamless environment for clients. The clinics goal is to remove barriers and improve access to care and reduce wait times to give our clients quicker access to services.
Your Responsibilities
As a Discharge Coordinator under CCBHC, you will work closely with the interdisciplinary team to deliver high quality, patient-focused services and be responsible for the effective coordination of patient discharges. This position is Monday-Friday, 4pm-12am. Additional responsibilities include:
- Facilitating safe discharge by coordinating all appropriate procedures, working in partnership with the interdisciplinary team and outside agencies to ensure that all plans are in place for the time of discharge, including transportation arrangements
- Discussing discharge plans with the patients and their families to ensure they understand the plans
- Acting as a liaison and facilitating discharge planning for patients with identified continuing care and complex needs
- Acting as a link to facilitate patients referrals in coordination with other health services
- Serving as a resource for questions related to the discharge processes
What You Need
- Bachelor's degree in a human services or healthcare related field. Degrees in other fields of study will be considered with relevant experience
- Relevant mental health care qualifications, certifications, licensure, or experience
- Clear knowledge and understanding of the patient discharge process
- Excellent verbal and written communication skills
- Ability to maintain confidentiality and handle sensitive or complex situations and conflict
- Knowledge of or experience working with EHR and patient databases
