Job Overview

The Case Manager is responsible for ensuring the general well-being of tenants through the delivery of high-quality services in a manner that is consistent with the mission, vision and philosophy of EKLC, as well as, the philosophy of assisted living. Must help keep the facility in compliance with applicable state laws and regulations.

The Case Manager is responsible in coordination with the Director, for the management and supervision of the associates and ensuring adequate associate staff scheduled and working for each of the shifts.

The Case Manager coordinates care across an episode and/or the continuum for tenants with complex problems and diverse needs. The Case Managers focus is to maintain tenants at an optimal level of health and to support self-care.

The Case Manager collaborates with physicians, nurses, nurse practitioners, social services, family members of tenants, EKLC Tenant Associates, EKLC Director, Area Agency on Aging and community agencies to deliver quality care and to achieve optimal outcomes.

Specific responsibilities include case assessment/screening, and assurance of timely services. Tenant outcomes are achieved through effective application of personal care plans, managed care concepts, appropriateness criteria, resource management, knowledge of community resources, and collaboration with other clinical disciplines. He or she works proactively to assist tenant with the coordination of services of physicians, nurses and other disciplines. The Case Manager facilitates personal care plan development with the Tenant Associates Activity Coordinator and Director.

Monday, Wednesday, Friday 8:30 am-5 pm, Tuesday & Thursday 8:30 am-2:30 pm and on call every other week by phone. Must stay in the state of Iowa when on call, per Iowa code; Only come in for significant change in condition, post hospital.

Bachelor's degree preferred, but not required.

Must be a licensed registered nurse with current R.N. license to practice nursing in the state where tenant care is administered. B.S.N. or

Case Management certification preferred but not required.

Certified Case Manager/Certified Professional in Health Care Quality (CPHQ) or must obtain within 24 months.

Three to five years related clinical experience.

Broad-based health care operational experience.

Valid State of Iowa driver’s license.

Must possess a high degree of interpersonal relations skills and capability of relating to a variety of people and personalities.

Considerable initiative, judgment and leadership skills are also required.

Must have the demonstrated ability to hire, train, direct and manage personnel.

Must possess strong organizational and time-management skills.
