Job Overview

MercyOne is seeking applicants to fill AmeriCorps member Community Health Worker (CHW) positions for sites in Clinton, Des Moines, Sioux City, and Waterloo. An AmeriCorps CHW is a frontline public health worker who is a trusted member of and/or has a remarkable understanding of the community served. AmeriCorps CHWs facilitate screening for social influencers of health (SIoH) and partner with patients to navigate the health and social service system.

Travel: In state travel during scheduled hours approximately once per month for training and education. Initial training may require one to two overnights in Des Moines, IA.

High school diploma or GED 18 years or older at the start of service
