About the Program
The Assertive Community Treatment (ACT) team is an effective, evidence based treatment program to provide in-home treatment, support, and rehabilitative care to persons with serious mental illnesses.
ACTs wraparound services aim to reduce the number of people who are in and out of hospitals for treatment or who are incarcerated or in residential care facilities. This is done mobilizing mental health and treating people out in the community. ACT teams are made up of experts, such as psychiatrists, psychologists, social workers, and employment specialists, who are on call 24 hours a day, seven days a week.
Your Responsibilities
As a Registered Nurse (RN) you will work as part of a multidisciplinary team to coordinate and provide whole health services to the clients assigned to the program. Responsibilities will also include:
- Participating in care coordination activities and case discussions for members
- Promoting access to behavioral health and physical health services and collaborating between these entities when needed
- Understanding and monitoring each members concerns and the types of resources needed
- Communicating the needs for health services to clients and staff
- Assessing client needs thoroughly and applying specific treatment modalities
- Assessing, monitoring, and evaluating the needs for health services to clients
*This part time position is scheduled to work ten (10) hours per week.
What You Need
- Graduate of accredited School of Nursing
- RN with current Iowa licensure in good standing pursuant to 655-Chapter 3
- Prior experience working with persons with severe and persistent brain health issues
- Knowledge on psychotropic medications, DSM-IV diagnosis, and co-occurring disorders
- Proficiency in time management and EHR documentation
- Demonstration of a professional commitment to providing services to medically underserved individuals
- Must be able to perform the essential functions of this position
